Required Forms

FOR FIRST TIME ATTENDEES:

Submit the following paperwork to Lt Col Michael Harding TWO WEEKS PRIOR TO THE COURSE DATE:

• 1. Completed online CAP registration at: Course Registration

  2. Completed online NRA registration for the course. See Course Dates for links to specific courses.

  3. Signed CAPF 60-80 – CAP Cadet Activity Permission Slip (Cadets Only) for the requested course.

  4. Signed copy of the Site Release Form (HRPC or OSS) for location you will attend. This is NOT required for academic courses.

1. Copy of your CAPF 160 – Member Health History Form. Required for live fire events (only needs to be submitted once for multiple events)

   1. Copy of your CAPF 161 – Emergency Information with a copy of your Insurance Card on the back. Required for live fire events (only needs to be submitted once for multiple events)

   2. Copy of your CAPF 163– Provision of Over the Counter Medication (if under 18). Required for live fire events (only needs to be submitted once for multiple events)

2. Cash or check payable to: Civil Air Patrol ($25 for classroom, $50 for live fire)

FOR PREVIOUS ATTENDEES:

Submit the following paperwork to Lt Col Michael Harding TWO WEEKS PRIOR TO THE COURSE DATE:

• 1. Completed online CAP registration at: Course Registration

1. Signed CAPF 60-80 – CAP Cadet Activity Permission Slip (Cadets Only) for the requested course.

   1. Signed copy of the Site Release Form (HRPC or OSS) for location you will attend if you have not already completed one for that site. If you have already submitted a Site Release form for the intended location, you do NOT need to submit another one.

   2. Copy of any medical forms (CAPF 160, 161, 163) if any data has changed since your last course. If no changes, you do NOT need to submit new forms. Remember you MUST have these forms on your person at AT ALL TIMES during these courses.

2. Cash or check payable to: Civil Air Patrol ($25 for classroom, $50 for live fire)

Forms may be brought into a Boca Squadron Meeting on Tuesday nights, sent via email to mharding@flwg.us, or mailed.
